Bulbar pressure test in glaucoma.

چکیده

THIS paper describes a relatively simple clinical test devised to estimate the facility of outflow of aqueous from an eye while external pressure is being applied to the globe; such a test is obviously of value in the investigation of cases of glaucoma. The fact that it is possible to reduce the tension in the eye by massage has been known for many years, and interest in the subject was revived during the latter part of the 19th century, when the invention of the tonometer enabled changes in the intra-ocular pressure to be measured with some degree of accuracy. Schiotz (1908) himself noted that if the tonometer were left too long on the eye the tension was reduced, and Polak-van Gelder (1911) stated that this fall in tension was due to the expression of aqueous from the eye, and that its magnitude depended on the ease with which the aqueous could escape. She also stated that a greater amount of massage was necessary to produce the same fall in tension in glaucomatous as in normal eyes, and she appears, in fact, to have been the first person to use massage with a tonometer as a diagnostic test. Since then numerous attempts have been made to develop the potentialities of this technique, but, though manyworkers agreed that in general the pressure did not fall so much in glaucomatous eyes, especially those which were already in a state of increased tension, they were unable to produce a standard minimal fall in tension below which limit an eye can be considered abnormal. Thomassen (1946) studied the effects of compressing the eye with various weights. Although he found that eyes with advanced glaucoma were not so easily compressed, he came to the conclusion that it was not possible by this method to detect any significant difference between the fall produced in normal eyes and that in glaucomatous eyes with normal tension. Consequently, most of his work was devoted to the study of the secondary rise in tension that occurs after compression. His monograph includes a very comprehensive review of all the previous work done on this subject. Moses and Bruno (1950) studied the fall in tension produced by leaving an electric tonometer on the eye for 2 minutes, and stated that the rate of

What is the Real Cause of Glaucoma?

Introduction: The term glaucoma refers to a group of diseases that have a characteristic optic neuropathy with associated visual field loss in common. Intraocular pressure has been considered to be the only causal factor for glaucoma and the only factor that can be manipulated to alter the course of the disease. But considering high intraocular pressure as the only factor responsible for glauco...

Evaluation of various factors relating to intraocular pressure and C/D ratio

Introduction: Intraocular pressure (IOP) is determined by three factors: the rate of aqueous humor production by the ciliary’s body, the resistances to aqueous out flow across the trabecular meshwork and the level of episcleral vessel pressure. IOP varies with a number of factors, including: age, sex, race, systemic disease and diurnal variation. The purpose of this study is measurement of IO...

Ciliary Body Tuberculosis Presenting with Attacks of Acute Intraocular Pressure Rise

Purpose: To report a rare presentation of ophthalmic tuberculosis. Case Report: A 19-year-old man was referred to the glaucoma clinic due to multiple attacks of increased intraocular pressure (IOP) in his right eye. IOP of the right eye was 50 mmHg, and glaucomatous damage of the optic nerve was present. In gonioscopy, an oval-black lesion was present in the ciliary body band on the temporal si...

A Case of Primary Congenital Glaucoma

An 11-year-old girl and a case of primary congenital glaucoma (PCG) with negative family history of glaucoma, was referred to our glaucoma service for uncontrolled intraocular pressure (IOP). Maximum tolerable medications and multiple glaucoma operations had failed to control her IOP. Review of the patient’s chart revealed an IOP of 30 mmHg and horizontal corneal diameter of 11.5 mm in both eye...

تعیین عناصر اطلاعاتی و مجموعه ی حداقل داده های مورد نیاز جهت ایجاد پرونده الکترونیک تحت وب بیماران مبتلا به گلوکوم

Background and Aim: Glaucoma, with an increasing pressure inside the eye, is one of the causes of blindness worldwide. The only glaucoma treatment is regular eye examination and control of intraocular pressure (IOP). The centralized information obtained from these examinations is an essential prerequisite for providing optimal healthcare which is possible by creating electronic records. Minimum...
